CIL-NET Presents… A National Teleconference & Webinar: Independent Living Skills Training for Transition to the Community

It can be difficult and time consuming to assist consumers moving back into the community from nursing facilities and other institutions. But don’t let that stop you from doing it!

IL-NET Logo - CIL-NET + SILC-NETSuccessful transitions from institutions back to the community often require coordination of all of your CIL’s core services. IL skills are especially important to make sure consumers that move home – stay there and live happily and independently. Our presenters will explain how to apply what you already know and how to expand on your IL skills training specifically for individuals who are moving to the community. Find out how you and your staff can assist consumers to identify, learn (or re-learn) the skills that will increase the chances of a successful transition.

Registration Fee: $75.00. Fee is per site (connection) and does not apply per participant; registrants are encouraged to gather as many individuals as desired to participate by telephone. 

Target Audience: The target audience members for this training are Executive Directors, Directors of Independent Living Services, and direct service staff of Centers for Independent Living who are new to the transition process.

Learning Objectives

Upon completion of this webinar, participants will have knowledge and resources that will enable them to:

  • Describe how IL philosophy and CIL staff support a successful transition process by reinforcing the peer-to-peer approach
  • Identify skills and services most often needed by individuals who are transitioning out of an institution and into the community
  • Explain the process for assessing an individual’s training needs prior to and after the transition
  • Assist individuals to develop a personalized training plan to ensure their successful transition and integration into community living

Meet Your Presenters:

Rebecca Payton has been with the Regional Center for Independent Living and the Center for Disability Rights, Inc. for twelve years. In 2014, RCIL received a BIP Innovation Grant to do nursing facility transition and diversion. With these funds, RCIL was able to employ three full time Transition Coaches who focused solely on our fifth core service. The results of this program were so successful that the Transition Coaching program was continued under CDR in April 2016, with new state IL funding. Rebecca currently oversees this nursing facility transition program and its companion program, the Open Doors Transition Center at CDR. Rebecca’s commitment to the mission and strong belief in advocacy is what successfully drives our transition programs.

Jane Schiele started working in the Independent Living Movement in 2000. In 2002 Jane coordinated the Nursing Facilities Transition Project for Colorado. Since then she has taught numerous classes for the ILRU including the ABC’s of Nursing Home Transition both on site and online, Orientation to Independent Living for New CIL Personnel, and Nursing Home Transition and Independent Living Skills. Jane currently works at the Center for People With Disabilities in Boulder where she manages the Transitions Program. It is the strength of the human heart and spirit that keeps Jane so deeply engaged in this work.
